  • Normally, SWOSU will not provide financial aid for a student to take a class at another institution. In some instances where there are extenuating circumstances, and the student cannot take a required course at SWOSU, we (Home Institution) will enter a Consortium Agreement with another school (Host Institution). Students should be enrolled in as many hours at SWOSU as they are at the host institution. If you are a SWOSU degree seeking student, enrolled at SWOSU and seeking a consortium agreement, please complete this form. Consortium Applications must be completed before the term they are intended for. We will notify you by SWOSU email whether your request is approved. The student understands this agreement cannot be completed if he/she is not making Satisfactory Academic Progress at SWOSU. Further, this coursework, like SWOSU work will impact subsequent SAP calculations. Within three weeks after the contracted period, the Student will provide the SWOSU Registrar's Office with an official copy of his/her academic transcript from Host school. SWOSU will not release official transcripts for students who have not provided a transcript from the Host Institution. Consortium Agreements should be completed before the first day of the term it is intended for.

  • By signing below, the Registrar's Office at Southwestem Oklahoma State University confirms the above named student to be enrolled as a regular student in a program of study leading to a *________ degree at SWOSU as indicated above by this student, for which the off-campus coursework given by the Host School will count. Upon receipt of an official academic transcript issued by the Host School, the SWOSU Registrar's Office will apply said coursework for transfer credit as appropriate.

  • This form requires a wet signature from the office of Academic Records so the form will need to be printed and emailed to the Office of Academic Records at academicrecords@swosu.edu or mailed to their office at the address below.
